When surgery is indicated

Usually first for stages I–II with no distant spread. For larger or aggressive tumors the board often recommends chemotherapy first — to shrink the tumor and preserve the breast.

Surgery first is typical when
Tumor under ~2 cm, clinically node-negative · DCIS · patient preference for immediate surgery after board review
The board may advise treatment first when
HER2-positive or triple-negative subtype · tumor over 2 cm or node-positive · inflammatory breast cancer

The two main operations

Lumpectomy + sentinel node

breast-conserving · 1–1.5 h · 1–2 nights

Tumor removed with margin; first nodes checked in-op. Followed by radiotherapy. Equivalent survival to mastectomy when eligible.

Mastectomy + reconstruction

2.5–4 h · 2–3 nights

Whole-breast removal with immediate implant or own-tissue reconstruction, one anesthesia. Onco-plastic is standard in Istanbul units.
